Right, it's what your mind perceives a doorknob to be. While not necessarily "pure," it is a strong image.

Touching the doorknob may have felt hyper-real, yes.

Sometimes when I'm at the dentist I have very noticeable hallucinations, but I never fall asleep and actually dream. I suppose that if I did, however, I might have a chance to lucid dream.

You could try alternatives, such as using a voice recording system instead of writing. That might be the small bit of convenience you need to get motivated.

I almost always dream as myself. I can't really remember any dreams off the top of my head where I was a different person or a different gender, but I feel like it's happened a few times.
